What are all of the Forms?

There are 10 main Forms in use at the ATF
  • Form 1 - (5320.1) Application to Make and Register a Firearm
    This form is used by individuals to manufacture and register NFA items themselves. For instance when registering a pistol as a SBR in order to add a stock to it legally. The cost of a Form 1 is $200 paid to the ATF regardless of what you are manufacturing.

  • Form 2 - (5320.2) Notice of Firearms Manufactured or Imported
    This form is used by licensed manufacturers to register NFA items they have made for sale to individuals, dealers, and governmental agencies. The cost of a Form2 is paid through the annual SOT of the FFL holder.

  • Form 3 - (5320.3) Application for Tax-Exempt Transfer of Firearm and Registration to Special Occupation Taxpayer
    This form is used by Special Occupations Tax holding dealers and manufacturers to transfer NFA items between themselves, FFL to FFL. The most common use of this form is when transferring NFA items to a dealer local to the buyer for transfer to the buyer on a Form 4. The cpst of a Form 3 is paid through the annual SOT of the FFL holder.

  • Form 4 - (5320.4) Application for Tax Paid Transfer and Registration of Firearm
    This form is used to transfer NFA items from currently registered owner to a new registrant. The most common use of this Form is when purchasing an NFA item from a licensed dealer with SOT to register the item to the buyer BUT is also used when stranfering NFA items between individuals in private sales. The cost of a Form4 is $200 for SBRs, SBSs, and Machine Guns or $5 for AOWs and is paid to the ATF by the buyer.

  • Form 5 - (5320.5) Application for Tax Exempt Transfer and Registration of Firearm
    This form is used to transfer unservicable firearms as curios or decorations, when transferring NFA items to governmental agencies like law enforcement or military, or when transferring NFA items to an heir upon the death of the original owner.

  • Form 6 - (5320.3A-D) Application and Permit for Importation of Firearms, Ammunition and Implements of War
    This form is used for the importation into the US of foreign manufacture items both under NFA purview and also non NFA items as well.

  • Form 7 - (5310.12) Application for Federal Firearms License
    This form is used to apply for an FFL to become a manufacturer or dealer of firearms and ammunition.

  • Form 8 - Application to Renew Federal Firearms License
    This form is used to renew an expiring FFL

  • Form 9 - (5320.9) Application and Permit for Permanent Exportation of Firearms
    This form is used when permanentaly exporting NFA items out of the country for international sale.

  • Form 10 - (5320.10) Application for Registration of Firearms Acquired by Certain Governmental Entities
    This form is used by state and local governmental agencies to register an unregistered NFA item for the possession and use of the agency.
